Braces or clear aligners? An honest comparison

Most comparisons of braces and aligners are written by whoever sells one of them. Here is the version we give in the chair.

Where aligners genuinely win

• Appearance during treatment. For an adult who deals with clients or is getting married in eighteen months, this is not vanity, it is the deciding factor.
• Cleaning. You take them out to brush and floss normally. Fixed braces make hygiene genuinely difficult, and eighteen months of difficult hygiene leaves marks on the enamel.
• No emergencies. No brackets coming loose, no wire poking the cheek on a Sunday.
• You can see the plan first. The projected movement is shown on screen before anything begins.

Where fixed braces still win

• Complex movement. Significant rotations, bringing down impacted teeth, large bite corrections — fixed appliances apply forces aligners struggle with.
• They do not depend on you. A brace works twenty-four hours a day because it cannot be removed. This is their single biggest advantage.
• Cost. Fixed treatment is usually less expensive.
• Children and teenagers. Twenty-two hours a day of compliance is a lot to ask of a fourteen-year-old.

The part nobody emphasises enough

Aligners only work while they are in your mouth. Twenty to twenty-two hours a day is not a recommendation, it is the mechanism. Patients who manage twelve hours do not get slower results — they get a plan that stops tracking, teeth that no longer match the trays, and a treatment that has to be re-planned.

Be honest with yourself about this before choosing, not six months in. If you know you will not wear them, fixed braces will give you a better result.

How long each takes

Broadly similar for comparable cases — the appliance is not what determines the duration, biology is. Teeth move at the speed bone remodels, and no system changes that. Anyone promising a dramatically faster result with the same movement is selling something.

Retainers, which are not optional

Teeth drift throughout life. Whatever moved them into position has to be followed by something that holds them there — for years, realistically indefinitely at night. Relapse after successful orthodontic treatment is common, and almost always down to retainers being abandoned. Budget for them from the start.

How to decide

Have the assessment before you have the preference. Records, photographs and radiographs will show whether your case is one aligners handle well, one that needs fixed appliances, or one where a combination is best. A clinic that recommends the same appliance to everyone is not assessing anything.
